Strengthening the Hospital–FQHC Partnership: Building Connected Systems of Perinatal Care

Pregnancy and the postpartum period extend well beyond the walls of the hospital. Strong partnerships between birthing hospitals and Federally Qualified Health Centers (FQHCs) are essential to improving maternal and infant outcomes, addressing health-related social needs, enhancing continuity of care, and reducing disparities.

This interactive learning session brings together hospital and FQHC leaders to explore opportunities for collaboration across the continuum of perinatal care. Participants will examine successful partnership models, identify operational and clinical considerations, discuss referral and communication pathways, and begin developing strategies to strengthen coordinated care within their communities.

Designed for multidisciplinary teams, this session emphasizes practical implementation, shared accountability, and actionable next steps that support high-quality, patient-centered maternal care.
